He espoused the combination of diagnosis and treatment, where one hand “listens” to the body while the other hand acts on it.

According to the principles of traditional Chinese medicine, external factors can invade the body to cause serious illness, but emotional disharmony within a person could also surface and cause illness in the physical body and the person’s behavior. The same illness or symptoms may have different causes and unless the correct cause is identified, healing will not be complete. Diagnosis is, therefore, considered very important in shiatsu.

“Listening” to the body in shiatsu terms means using not just the palms, fingers and thumbs but all sensory organs of the practitioner in detecting any disharmonies in the patient’s body energy. The practitioner assesses the patient’s outward appearance and checks wrist pulses as well as the state of the tongue. He palpates the abdominal area and the back, feeling for stiffness or slackness that may show signs of the state of energy distribution in the body. He evaluates the patient’s odor, observes the sounds made by the body and sifts through the patient’s responses to find psychological factors that may be contributing to the disharmony.

Shiatsu massage treats the body as a whole, stimulating the body’s natural healing ability and the immune system in order to restore the normal functions of the circulatory system, nervous system, skeletal system, muscles and internal secretions. Beyond that, it also aims to balance and align the mind and body in harmony.

To practice shiatsu in Japan, practitioners need to be licensed by the Ministry of Health, Labor and Welfare. To be licensed, they first need to have studied and passed a three-year program of training in a university or college authorized by the Ministry and passed a national examination.

Shiatsu has spread throughout the world and various countries have established their own shiatsu licensing requirements. Most require two to three years of specialized study with clinical practice, plus oral and written examinations covering both western anatomy and physiology and traditional Chinese medicine.

In the United States, the American Organization for Bodywork Therapies of Asia (AOBTA) is working with the National Certification Council for Acupuncture and Oriental Medicine (NCCAOM) to achieve standardization in educational requirements for Asian bodywork therapy, including shiatsu. They are introducing mandatory standardized national board certification requiring primary discipline training, training in anatomy and physiology, traditional Chinese medicine training, elective coursework and student clinic training.
